BLEVINS DIESEL

HOLD HARMLESS and LIABILITY WAIVER

NOTICE: The item described below is intended for use only by experienced
mechanics, fabricators, or welders who have the knowledge, skill, and equipment to
safely assemble and use it. This product is not certified, tested, or approved for safety
by any agency. Improper assembly or use could result in serious injury, death,
and/or property damage.

Item:

Cut steel components intended to be assembled by the purchaser into an adapter for
use between a truck differential/transmission/head and/or component and a transmission jack or any lifting device.

AS-IS CONDITION & NATURE OF ITEM

The above-listed components are raw steel pieces, cut to size, and are not a finished or
assembled product. They are provided strictly in as-is condition, without any warranties
express or implied—including but not limited to warranties of merchantability, fitness for a particular purpose, or safety.

USER ASSEMBLY & RESPONSIBILITY

The purchaser is solely responsible for any welding, fabrication, assembly, or modifications, as well as the safety, quality, and suitability of the finished product. Seller does not supervise, inspect, or certify the purchaser’s assembly or workmanship.

ASSUMPTION OF RISK

The purchaser acknowledges that welding, assembling, and using the finished adapter
involves significant risks, including but not limited to burns, mechanical failure, equipment damage, personal injury, or death. The purchaser voluntarily assumes all such risks and is solely responsible for ensuring proper design, assembly, and safe operation.

RELEASE & HOLD HARMLESS

The purchaser releases, discharges, and agrees to hold harmless the Seller, its owners,
employees, and agents from any and all claims, damages, losses, liabilities, or expenses whether caused by negligence or otherwise arising from or related to the receipt, assembly, use, or misuse of the components or any finished product derived from them.

INDEMNIFICATION

The purchaser agrees to indemnify and defend the provider from any claims brought by
third parties resulting from the purchaser’s possession, assembly, modification, or use of the components or any finished adapter.

ACKNOWLEDGEMENT

The purchaser affirms they have read and understood this waiver, are aware of the risks, and accept full and sole responsibility for the assembly and use of the product.
